Pineland Aesthetic Medicine is a telehealth medical practice and is not a substitute for your primary care provider. Patients remain responsible for routine care, preventive screenings, and other primary care needs.

We do not provide emergency services. For an urgent or emergency condition, call 911 or seek immediate care from your local emergency department. Patient portal messages are not continuously monitored and should never be used for urgent concerns.

How soon can I get started?

All consultations are free. Wellness visits are often available the same day or next day, while other appointments are typically available within 7–10 days.

Please complete all required intake paperwork in your patient portal before your appointment to prevent delays.

Do I need insurance?

No. Pineland Aesthetic Medicine is a cash-pay, membership-based telehealth practice. We do not accept insurance or submit claims to insurance companies.

A 10% military discount is available for qualifying patients.

Understanding Our Payment Structure at Pineland Aesthetic Medicine

When you enroll in a Pineland program, you are paying for participation in a comprehensive medical program—not simply for medication.

Program fees support medical evaluation, individualized treatment planning, provider oversight, ongoing guidance, monitoring, follow-up, and administrative support. Medication may be included when appropriate for the specific program.

Prescriptions are never guaranteed. Treatment and prescribing decisions are based on your clinical evaluation, safety, monitoring, applicable regulations, and independent provider judgment.

What is the Payment Policy?

Program Fees & Payment

Pineland Aesthetic Medicine is a cash-pay, membership-based medical practice. Program fees are paid in advance or on a recurring basis according to the program selected.

A valid payment method must remain on file for recurring programs. Program fees are for participation in your medical program and are not based on medication volume or a specific number of prescriptions.

Declined Payments

Failed or declined payments may result in suspension of care until the outstanding balance is resolved. Outstanding balances may be referred for collection.

Refunds & Chargebacks

Payments are non-refundable once services are rendered or you are enrolled in the program, according to the terms of your signed Program Agreement.

Patients should contact Pineland directly with any billing concern before initiating a chargeback or payment dispute.

Cancellation & Prescription Treatment

Cancellation requests must be submitted in writing through the secure patient portal. The 30-day notice period begins on the date your written cancellation request is received.

If you are receiving prescription treatment, submitting a cancellation request does not mean you should simply stop your medication. The practice will contact you to schedule an off-boarding provider appointment when appropriate so your treatment can be discontinued, transitioned, monitored, or tapered under medical supervision based on your medication and individual clinical needs.

Continued prescribing or refills during the cancellation period require completion of the required off-boarding visit when clinically indicated.

Your account remains active and scheduled billing continues during the 30-day notice period. Payments made during this period are non-refundable.

Pineland does not offer medication pauses, temporary holds, or temporary stops. A request to pause or temporarily stop treatment is treated as a cancellation.

To avoid another billing cycle, your written cancellation request must be received at least 30 days before your next renewal date.

Restarting after cancellation requires a new provider visit and clinical approval.

Appointment Cancellation & No-Show Policy

Appointment cancellations or rescheduling requests must be submitted through the secure patient portal at least 24 hours before the scheduled appointment.

A no-show includes failure to attend a scheduled appointment without prior notice, cancellation or rescheduling with less than 24 hours' notice, or arrival more than 15 minutes late at the provider's discretion.

First no-show: A $50 missed appointment fee may be assessed at the practice's discretion and may be waived.

Second no-show: A $50 missed appointment fee will be assessed, and medication refills will be placed on hold until the rescheduled appointment is completed.

Third no-show: A $50 missed appointment fee will be assessed, and continued care may be terminated at the discretion of Dr. Karla Gochenour, DNP, FNP-BC.

Missed appointment fees are non-refundable and may be charged to the payment method on file.

Do you offer online Telehealth visits?

Yes. Pineland Aesthetic Medicine provides care through telehealth.

At the time of each appointment, you must be physically located in a state where Dr. Karla Gochenour, DNP, FNP-BC is actively licensed: North Carolina, Virginia, Colorado, Connecticut, or Florida.

Treatment and prescribing laws vary by state, so certain services or prescriptions may not be available in every licensed state.

Medical Weight Loss

What weight loss medication options are available?

We offer multiple medical weight-management options that may include injectable or oral medications, stimulant or non-stimulant treatments, GLP-1 medications, and other therapies that address appetite, cravings, or emotional eating.

Treatment is individualized based on your medical history, goals, contraindications, response to treatment, and provider judgment.

Some medications may be compounded when legally permitted and clinically appropriate. Compounded medications are not FDA-approved in their compounded form.

Do you prescribe Wegovy®, Ozempic®, Rybelsus®, or Mounjaro®?

Prescription weight-management and metabolic medications may be considered when medically appropriate. GLP-1 medications are not the only option available.

Some medications may also be compounded when legally permitted and clinically appropriate for an individual patient. Compounded medications are not FDA-approved in their compounded form.

During your free consultation, available treatment options can be discussed based on your goals, medical history, safety considerations, and individual needs.

How do GLP-1s work for weight loss?

GLP-1 medications act on pathways involved in appetite, fullness, digestion, and glucose regulation. They may reduce hunger, help you feel full longer, and reduce overall food intake.

Response and tolerability vary from person to person, and medication is only one part of a medical weight-management plan.

Do you prescribe Phentermine for weight loss management?

Phentermine is a controlled medication with potential for abuse and dependence. Our practice generally prefers alternative weight-management options.

Whether phentermine or another treatment is appropriate can be discussed during your individual consultation.

Are Weight Loss medications given by an injection?

Not always. Both injectable and oral weight-management medications may be available.

During your consultation, your preferences, medical history, treatment goals, and clinical needs will be reviewed to determine which options may be appropriate.

How much weight can I expect to lose?

Weight-loss results vary. Your response depends on the medication or treatment selected, your medical history, nutrition, activity, adherence, tolerability, and other individual factors.

Weight-loss treatment is not one-size-fits-all, and no specific amount or rate of weight loss can be guaranteed.

Where does my medication come from?

At Pineland Aesthetic Medicine, we collaborate with licensed partner pharmacies to help patients receive prescribed medications and support order fulfillment.

You may contact the dispensing pharmacy directly regarding prescriptions, medication, or order fulfillment. If you are unsure which pharmacy is fulfilling your treatment plan, contact Pineland for assistance.

When Your Medication Arrives

  • Open the package promptly.
  • Review the medication label and packaging.
  • Follow all storage instructions exactly.
  • Confirm that all expected supplies are present.
  • Contact the pharmacy using the information on the medication label if something is missing or damaged.
  • Contact your provider if you have questions before use.

Always follow the instructions provided by your dispensing pharmacy and medical provider.

Testosterone Therapy

What if my PCP says I don't qualify for TRT?

You may request a free independent consultation with Pineland Aesthetic Medicine.

Testosterone therapy is not determined by insurance coverage or patient request alone. Dr. Karla evaluates your symptoms, medical history, appropriate laboratory results, possible causes of testosterone deficiency, contraindications, treatment risks, and other relevant clinical factors before determining whether treatment is appropriate.

TRT PROGRAM FEE $179

Program fee. Laboratory testing is separate when clinically appropriate.

What if I’m already on TRT, do I need to lower my Testosterone levels to join?

No. You should not intentionally lower your testosterone level simply to transfer care.

If you are transferring from another TRT practice, Dr. Karla will review your current treatment, laboratory results, symptoms, medical history, and monitoring.

Transfer of care does not guarantee continuation of the same prescription, dose, or treatment plan. Any changes are based on your individual clinical evaluation.

Are labs included in the monthly cost?

No. Laboratory testing is separate from the TRT program fee.

When laboratory testing is clinically appropriate through Pineland's lab program, the lab fee is $179.

You may also provide eligible laboratory results obtained elsewhere for provider review at no additional review fee, or labs may be ordered through our discounted LabCorp network when appropriate.

When do I need to have labs drawn on TRT?

Your initial follow-up after beginning testosterone therapy is typically scheduled around 8 weeks after treatment begins. Ongoing laboratory and follow-up requirements are determined by your provider based on your treatment and clinical needs.

Required laboratory testing and follow-up must remain current. Refills will not be issued when required monitoring is overdue.

You can monitor laboratory due dates through your patient portal. Once results are received, a team member may contact you to schedule your review appointment.

Medication & Shipping

How do I track my package?

Shipping and tracking procedures vary by pharmacy. Some pharmacies provide tracking information directly, while others may not.

For the most current information about your order, contact the dispensing pharmacy directly.

Keep your shipping address current and retrieve delivered medication promptly. Temperature-sensitive medication should be stored according to the pharmacy's instructions.

Inspect your shipment when it arrives. If it is damaged, incomplete, lost, or appears compromised, contact the dispensing pharmacy and Pineland promptly and retain the packaging if a carrier claim is needed.

Lost or stolen medications are not replaced. When applicable, the patient is responsible for initiating the carrier claim and providing the requested documentation.
